B) Briefly why you want me to help you. (EI Impending death, uncomfortable with past deeds, want to live a more rewarding life, etc.)

C) Current beliefs (IE Religion or spirituality) Note-I don’t try to convert anyone to any other faith structure but I do explain what in your dogma set is not only incorrect but harmful to your eternal future.

D) Whatever else you think I should know about you, but I will ask if you haven’t provided me with the information that my pre-judgment of you requires.
